Are you an Association Administrator wanting to add a new Club to MySideline? 

Before you complete the form, please review the following:

Does this Club already exist in MySideline?

Review the database prior to requesting a new Club to be created in MySideline. Each State League has a representative who will be able to assist with the MySideline database and its structure. We encourage you to consult with them and other colleagues prior to requesting a new Club.

If a new Club is required, please complete the below form and a member of our friendly Support Team will contact you shortly.
